Nurse-Led Care at Home

Some health needs call for more than personal care, but that doesn’t mean having to leave the comfort or familiarity of your own home.

Nexus Care Services now offers nurse-led care for people with clinical needs, delivered by a team overseen by a named Clinical Lead and registered nurses, working closely with GPs, hospital teams and case managers.

Whether someone is being discharged from hospital, managing a long-term condition or needs ongoing clinical support, our nurse-led team brings the same safety and oversight you’d expect in a clinical setting into your home.

We support people and families across Sutton Coldfield, Tamworth, Lichfield and the wider West Midlands.

Who nurse-led care is for

Nurse-led care at home covers a wide range of conditions and requirements that exceed what can be provided by standard domiciliary care.

We can support:

Families and individuals

People returning home from hospital who need a safe step down, people living with a long-term or complex condition and families who want consistency, dignity and control over the care a loved one receives.

Healthcare professionals

Hospital discharge teams, Continuing Healthcare assessors, case managers, occupational therapists, solicitors and financial advisers managing care plans for clients who need a clinically licenced care provider they can refer into with confidence.

What our nurse-led services cover

Our nurse-led team supports a wide range of clinical needs at home. Each area below is led by registered nursing staff and built around a personalised care plan, following an initial clinical care assessment.

Hospital discharge support

Safe, swift transitions from hospital to home, with clinical monitoring in place during the early and often most vulnerable days of recovery.

Complex care at home

Specialist nurse-led support for people living with spinal injury, acquired brain injury, MS, MND and other complex neurological conditions, including support for ventilated clients.

Frailty care

Nurse-led support for older adults living with dementia or frailty, focused on keeping people safe, well and independent at home for longer.

Long-term condition management

Ongoing clinical support for diabetes and other long-term conditions, including insulin administration, helping people manage their health day to day.

Community nursing

Scheduled nurse visits, clinical reviews and wound care delivered at home rather than a clinic.

Catheter care

Professional, dignified catheter management carried out by trained nursing staff.

PEG and enteral feeding support

Safe administration and monitoring of tube feeding for those who need it, with full clinical oversight.

Medication management and clinical observations

Administration and oversight of medication and routine clinical observations, reducing risk and giving families reassurance.

Clinical assessments

Initial, discharge and ongoing risk assessments carried out by registered nursing staff, forming the basis of every care plan.

Admission avoidance care

Proactive clinical support designed to identify problems early and prevent unnecessary hospital stays.

Our clinical team

Every nurse-led package sits under a single line of clinical accountability.

A named Clinical Lead oversees our registered nurses, who in turn oversee our TDDI Practitioners, all of whom are competency-assessed before they deliver clinical care.

This structure means one point of responsibility for every decision, not a rotating cast of carers.

Nexus is registered to deliver Treatment of Disease, Disorder and Injury (TDDI) services, the CQC category covering this level of clinical care, giving professional referrers the assurance that comes with formal clinical governance.

We will always try to ensure that we match your care team to your loved one’s requirements, so that we create relationships that deliver dignity.

How nurse-led care works

Get in touch

Whether you’re a family member, a hospital discharge team or a case manager, the process starts with a conversation about the support that’s needed.

Clinical assessment

A registered nurse or our Clinical Lead visits to assess clinical needs, risks and personal goals, giving us a clear picture of what good care looks like for that person.

A personalised care plan

We build a plan around the individual’s clinical needs, preferences and home environment, set within a single care pathway that can flex from home care through to complex nurse-led care as needs change.

A matched care team

Every person is supported by a clinically supervised team, from TDDI Practitioners to registered nurses, all overseen by our Clinical Lead.

Ongoing clinical review

Care doesn’t stand still and so we carry out regular clinical reviews so the plan keeps pace with changing needs, with full governance and reporting behind every package.

The continuous care journey

We are in a unique position to support people throughout their journey in care, to ensure that they remain with a provided that not only understands their needs, but also the little things that matter most to them.

With us we can continue to support you along the following pathway:

Home Care

1

Live-In Care

2

Hospital Discharge Support

3

Nurse-Led Care

4

End-of-Life Support

5

We will work with you as your long-term care provider to ensure that you achieve a greater consistency of service.

The difference it makes

Every nurse-led package is measured against outcomes that matter, not just hours delivered.

We track:

  • Hospital admissions avoided
  • Hospital discharges supported
  • Readmissions prevented
  • Falls and pressure ulcers prevented
  • Early deterioration identified
  • Hospital days saved
  • People remaining independent at home

We use the latest technology, including cloud-based platforms, to monitor this information and share it with families and healthcare professionals.

Why families and professionals choose Nexus

Nexus is a CQC-rated, multi-award-winning care provider with an established reputation across Sutton Coldfield, Tamworth, Lichfield and the West Midlands, now extending that same standard of care into nurse-led clinical services.

Because we offer one continuous pathway, from home care through to live-in care and nurse-led complex care, people don’t need to move providers as their needs change.

The same team, the same governance and the same standards follow you throughout.

Our workforce is competency-assessed and developed through a structured clinical career pathway, which means greater consistency for the people we support and confidence for the professionals who refer to us.
